To register or make an appointment for physical or occupational therapy rehabilitation you must obtain a referral from your family doctor, nurse practitioner, dentists, podiatrist or chiropractor. Then call 1-888-923-7781 for the clinic location nearest you. When calling, please inform our staff of the type of health insurance you have so we can verify and assure your coverage for your upcoming visit. You may print and fill-out new patient forms from the link below, or request that we e-mail our initial visit forms so that you may have them filled out and bring them with you for your first visit.
